Athletic Eligibility 2019-20
Athletic Eligibility Forms
All student-athletes for the 2019-20 school year must be athletically eligible prior to tryout dates or the first practice (for those that do not have tryouts). Each student must turn in the Physical Clearance Form, Medical History Form, CIF Code of Ethics, and the Athletics Handbook Signature Page (is currently being revised for 2019-20). All forms must be turned in to the main office.
Located in the San Fernando Valley portion of the City of Los Angeles in Northern Los Angeles County. de Toledo High School is accredited by the California Association of Independent Schools (CAIS), the Western Association of Schools and Colleges (WASC) and Builders of Jewish Education (BJE,) and financially supported by the Jewish Federation of Greater Los Angeles and the Jim Joseph Foundation.
